Medical Confidence is a growing healthcare navigation company based in Richmond Hill, Ontario who is fully aligned with the Canadian Health Act, and works in partnership with patients, physician specialists and family practitioners. Medical Confidence’s mission is to EMPOWER patients to be informed and active participants in their healthcare decisions, by helping them understand their medical condition and access the most appropriate care in a timely manner. This can include assessing patients, finding the right doctor and/or social services programs, completing testing and imaging, receiving a referral, booking appointments and treatments, etc.

We are currently looking for the right individual to join our team as a full-time Mental Health Patient Navigator. We offer flexibility in the workplace with the opportunity to work from a home office, and career growth opportunities in our growing organization. There is a potential of traveling to one of our corporate client’s office in Montreal once per month.

Summary/Objective
The ideal candidate will be a member of Provincial Order/College of Social Workers or Social Services Workers. Previous experience working with mental health patients which includes facilitating communication between patients and their medical practitioners, assessing patient’s needs, advocating on behalf of the patient, and identifying appropriate interventions that promote a greater level of functioning for the patient. Motivational interviewing training and experience, and suicide prevention training are both assets.

The applicant needs to be persistent, tenacious, curious, assertive, creative, able to multi-task through a strong sense of being able to prioritize competing priorities. In most cases, navigation requires creative thinking in a healthcare system where patients frequently encounter barriers throughout their journey. They must also thrive in a fast-paced environment, and be able to work both independently, as well as part of a team.

Fluency in both French and English is a must.

Roles and Responsibilities

  • Providing exceptional patient service;
  • Communicating with our patients, corporate clients, nurses, and doctors;
  • Capture patient evaluation/assessment using our case management system throughout the continuum of care
  • Research governmental programs applicable to mental health other medical conditions (across provinces);
  • Entering patient information and reports using various company technology systems;
  • Ensuring patients access the services they need in order to achieve their goals and recovery including scheduling appointments;
  • Collecting medical records from doctors in a timely manner; and
  • Other duties as required

Qualifications

  • Member of College/Order of Social Workers or Social Services Workers
  • 2-3 years work experience working within the Canadian healthcare system
  • Fluent in both English and French

Work Environment
This job operates in a work from home environment and occasional onsite meetings with our corporate clients.

Work Authorization/Security Clearance
Government clearance will be required for one of our federal clients.
